CHILD AND ADOLESCENT CONSULTATION – $750

• Pre- appointment Paperwork filled out by child (if able to do so) 
• Pre- appointment Paperwork filled out by caregivers 
• Review of pertinent old records and recommendations 
• 90 minute consultation with entire family 
• 60 minute consultation with child 
• 60 minute consultation with caregivers 
• 60 minute appointment to go over results and recommendations 

Recommendations/results session includes going over the following:

• Diagnosis/possible diagnosis
• Thorough medication recommendations to share with primary care physician
• Plant medicine recommendations if interested
• All of these will also be received in a report 

CHILD AND ADOLESCENT CONSULTATION + EDUCATIONAL RECOMMENDATIONS – $1350

($500 discount than paying for a psychiatric evaluation and Educational Consultation separately )

• Everything included in child and adolescent consultation
• Review of any psychological testing, school testing, and IEPs
• Additional education specific recommendations 
• Attendance of one IEP meeting (if desired)

ADULT CONSULTATION – $650

• Pre- appointment paperwork filled out by patient 
• Review of pertinent old records and medications 
• 2 hour initial consultation 
• 1 hour appointment to go over results and recommendations
